Fever before a rash: what to know

Fever followed by a rash can occur with viral illness, but the pattern and the person's condition guide urgency. Note when the fever began, when the rash appeared, and whether the person is drinking, breathing comfortably and responding normally.

A rash that appears as a fever settles can accompany illnesses such as roseola or chickenpox. A fever with a hot, painful, spreading area of skin may point toward cellulitis. Cough, wheeze or a harsh voice can suggest a respiratory infection such as bronchiolitis, croup or COVID-19. The rash's colour, texture, location and whether it fades when pressed are useful details, but they do not identify the cause alone.

Offer frequent fluids and let the person rest in light clothing. Check temperature with a reliable thermometer and record changes rather than judging fever by touch. Avoid scratching, overheating and sharing towels. Use fever medicine only as directed for the person's age and health; never give aspirin to a child. Keep someone with a new unexplained rash away from close contact until a clinician advises about contagiousness.

Get urgent medical help for trouble breathing, unusual sleepiness, confusion, a seizure, severe headache, stiff neck, repeated vomiting, signs of dehydration, or a rash that does not fade under pressure. Seek prompt assessment for a rapidly spreading or painful rash, or fever in a very young baby. Contact a clinician if the fever persists, returns, or the person is worsening even when the rash looks mild.

Could the timing and appearance fit a contagious infection? Does the rash need examination or testing? Which fluids and fever medicine are suitable for this person? How should we monitor breathing, hydration and alertness at home?
