Fever and red spots: what they may mean

Fever with red spots needs attention because the rash pattern, general illness, and speed of change help guide the next step. Red spots may be caused by a viral illness, skin infection, or an inflammatory reaction. The spots can be flat or raised, itchy or tender, and may appear on one area or spread.

Chickenpox commonly presents with fever followed by an itchy rash that develops in stages. Cellulitis presents with a painful, warm, expanding area of red skin, often on one limb. Respiratory illnesses such as COVID-19, croup, bronchiolitis, acute bronchitis, or a common cold may occasionally occur alongside a rash. Brucellosis can cause fever and feeling unwell, especially after relevant animal or unpasteurised dairy exposure. A rash alone cannot identify the cause.

Check the temperature with a reliable thermometer and note when the spots began, where they started, and whether they blanch when pressed. Encourage fluids and rest, and choose loose clothing. Do not scratch, squeeze, or apply a new medicated cream before advice. Avoid close contact with people who are pregnant, very young, or immunocompromised until a clinician has assessed a feverish rash that could be infectious.

Seek emergency help if the spots do not fade briefly when pressed, spread rapidly, or occur with confusion, severe drowsiness, breathing difficulty, a stiff neck, fainting, or blue or grey lips. Arrange same-day medical assessment for a child who looks very unwell, persistent vomiting, severe pain, or signs of dehydration such as very little urine. Take a clear photo of the rash if it changes before assessment.

Ask which causes fit the rash's appearance, whether testing is needed, how long to avoid school or work, and which symptoms should trigger urgent review. Mention recent travel, new medicines, insect bites, sick contacts, vaccinations, and animal or food exposures.
