Fever and a rash on the lips: what to know

A lip rash may appear as red or swollen skin, small blisters, crusting, cracks, or sores. Fever suggests that the body may be responding to an infection, although the rash can also be irritated by saliva, dry skin, a new product, or repeated rubbing. Viral illnesses can cause fever and mouth or lip changes; chickenpox may cause an itchy blistering rash elsewhere as well. A spreading, hot, tender area around the lip can point to a skin infection such as cellulitis. The rash’s location, texture, timing, and any mouth, throat, or breathing symptoms help a clinician judge the cause.

Rest and drink fluids in small, regular amounts. Keep the lips clean and avoid picking, squeezing, or peeling the rash. Use a plain, fragrance-free emollient on dry outer skin, but do not put household remedies or leftover prescription creams on blisters or open sores. Avoid sharing cups, towels, lip products, or close face-to-face contact until an infectious cause has been considered. Follow the label for any fever medicine, checking with a pharmacist or clinician first for a child, pregnancy, allergies, or other medicines. Take a clear photo and note when the fever and rash began.

Get urgent help if there is trouble breathing or swallowing, swelling of the tongue or face, confusion, severe weakness, a rapidly spreading rash, purple or non-blanching spots, or signs of dehydration. Arrange prompt medical assessment for a high or persistent fever, severe pain, pus, marked warmth, eye involvement, repeated vomiting, or a person who is very young, elderly, pregnant, or immunocompromised. A clinician can examine the lips and mouth and decide whether testing or treatment is needed.
