Fever and rash around the eyes: what to know

Fever with a rash around the eyes needs attention because swelling, pain, or eye changes can signal a more serious infection. The appearance may also fit a viral illness or irritation, so the rash alone cannot identify the cause.

Note whether the skin is red, blotchy, raised, blistered, warm, or tender. A rash limited to the eyelids may follow irritation, while spreading redness, warmth, and swelling can occur with cellulitis. Viral illnesses such as chickenpox or COVID-19 can cause fever and skin changes elsewhere. The presence of cough, sore throat, vomiting, headache, or eye discharge gives a clinician useful context.

Offer regular fluids and allow rest. Keep the area clean and avoid rubbing, new creams, or eye makeup. Check the temperature with a reliable thermometer and record when the fever and rash began. Do not place steroid or antibiotic cream close to the eye unless a clinician has advised it. Reduce contact with others while a contagious infection is possible, especially if blisters or a widespread rash develop.

Arrange prompt medical assessment for fever with increasing eyelid swelling, marked tenderness, warmth, pus, or redness spreading across the face. Seek urgent help for eye pain, blurred or double vision, difficulty moving the eye, severe headache, confusion, unusual sleepiness, breathing difficulty, or a rash that does not fade when pressed. A clinician may need to examine the eye and skin directly.

Ask whether the rash pattern suggests a viral illness, skin inflammation, or cellulitis; whether the eye itself needs examination; and which fever-relief medicine is suitable for the person who is ill. Mention recent medicines, insect bites, sick contacts, travel, and any change in vision.
