Fever of unknown origin: what should you know?

Many causes can produce prolonged or unexplained fever. They include infections that are difficult to localise, inflammatory conditions, medication reactions, and less common disorders. Symptoms can guide the search: cough may point toward Acute Bronchitis or another airway illness; a skin lesion may suggest Cellulitis; travel, animal, or food exposure may raise questions about Brucellosis. Common Cold, COVID-19, Bronchiolitis, Croup, or Chickenpox may be considered when their characteristic respiratory or skin features are present.

The clinician will ask about the onset, daily pattern, travel, contacts, medicines, immune health, dental symptoms, urinary changes, abdominal symptoms, rashes, and joint complaints. Examination and selected tests are guided by these findings rather than performed as a single identical package for everyone.

Record measured temperatures, the time of day, chills or sweats, medicines taken, and any new symptoms. Keep drinking fluids and rest while arranging medical review. Do not start leftover antibiotics or stop prescribed medicines without advice. Bring travel history, vaccination information, medical records, and a complete medication and supplement list.

Seek urgent help for confusion, severe weakness, breathing difficulty, chest pain, a stiff neck, severe abdominal pain, repeated vomiting, serious dehydration, a rapidly spreading rash, or difficulty waking. Arrange prompt review if fever persists, returns after improvement, or occurs in pregnancy, infancy, older age, or with reduced immunity.

Ask what findings are most concerning, which tests are being considered and why, whether any medicines could be contributing, how to monitor at home, and exactly when to return or seek emergency care.
