Fever but cold hands and feet

During a fever, the body can reduce blood flow to the hands and feet while it raises core temperature. Chills, shivering and cool fingers or toes can result. Cool surroundings, damp clothing and dehydration can add to the sensation. The temperature of the trunk may therefore feel different from the extremities.

Check for pale, blue or mottled skin, slow responses, weakness, dizziness, breathing changes and whether the person can drink. These findings provide more useful safety information than cold feet by themselves. A fever can accompany respiratory infections such as a common cold, but symptoms and examination are needed to identify the cause.

Take and record the temperature, offer fluids, and use socks or light layers for comfort. Keep the person away from extreme heat, heating pads and ice. Recheck colour, breathing and alertness after the shivering eases. Use fever medicine only according to the product instructions or a clinician’s advice.

Call emergency services for blue or mottled skin, severe breathing difficulty, confusion, collapse, a seizure, or a person who cannot be woken normally. Obtain urgent medical advice for a child who is floppy, refusing fluids or urinating much less. Arrange review if the fever lasts, returns after improving, or the hands and feet stay unusually cold with worsening symptoms.
