Fever but cold hands: what it means

When the body is raising its core temperature, blood vessels in the hands can narrow and the person may shiver. The hands may feel cool while the chest or forehead feels hot. This often changes as the fever settles. Room temperature, dehydration and prolonged inactivity can also make hands cool.

Look at skin colour and behaviour as well as touch. Blue, grey, very pale or mottled fingers, marked weakness, confusion, unusual sleepiness, or fast or difficult breathing are more concerning than cool hands alone. A thermometer gives more useful information than judging temperature by the skin.

Measure the temperature, offer small frequent drinks, and use comfortable layers. Remove heavy blankets if the person is sweating. Let shivering pass without cold baths or ice. Check breathing, alertness, urine output and hand colour regularly, and write down changes for a clinician.

Seek emergency care if cold hands occur with blue or mottled skin, confusion, collapse, severe breathing difficulty, a seizure, or inability to wake the person. Seek prompt advice when a child is unusually floppy, drinking poorly, or passing much less urine. Contact a clinician if fever persists, keeps returning, or the hands remain markedly cold despite warming and fluids.

Ask whether the temperature pattern fits chills, how often to check hydration and colour, and what threshold or symptom should trigger urgent review.
