Fever followed by a rash: what to know

Many viral illnesses cause fever before a rash becomes visible. Chickenpox often produces itchy spots that evolve into fluid-filled blisters, while a Common Cold or COVID-19 may cause fever with respiratory symptoms and sometimes a rash. Cellulitis more often causes a local area of skin that is painful, warm, swollen, and red. A rash after starting a medicine can represent a reaction and should be discussed promptly. The timing, spread, itch, pain, blistering, and whether the rash fades when pressed help a clinician assess it.

Take a clear photograph in good light and note when the fever and rash began, any new medicine, and recent contacts. Encourage fluids and rest, keep nails short, and use loose clothing. Do not scratch blisters or apply a new cream without advice. Avoid close contact with pregnant people, newborns, and anyone with weakened immunity until an infectious cause has been considered.

Get emergency help for a rash that does not fade under pressure, breathing difficulty, facial or tongue swelling, confusion, a stiff neck, seizure, or a child who is hard to wake. Seek same-day advice for rapidly spreading redness, painful blisters, skin peeling, a rash involving the eyes or mouth, or fever that returns after improvement. A clinician should review a new rash after medicine even if it seems mild.

Ask whether the pattern suggests an infection, medicine reaction, or skin inflammation; whether you should avoid work, school, or contact with others; and which treatments are suitable. Bring the medicine list and photographs showing how the rash changed.
