Fever and rash around the mouth: what should you know?

The pattern can accompany a viral illness, especially when there is sore throat, cough, runny nose, mouth discomfort, or a rash elsewhere. Saliva, lip licking, food irritation, eczema, and contact with a new product can inflame the skin around the lips, although these causes do not explain every fever. Blisters, crusting, painful sores, swelling, or a rash that spreads provide useful clues. A photograph and a timeline can help a clinician compare changes over time.

Offer fluids and rest, and use a suitable fever medicine only as directed for the person’s age and health. Wash hands, avoid sharing utensils, and do not scratch or pick the rash. Gently clean the area with water, pat it dry, and avoid fragranced creams or newly introduced products. Do not apply steroid, antibiotic, or antiviral cream unless a clinician recommends it. Keep children away from vulnerable contacts until the cause is clearer if a contagious illness is possible.

Call emergency services for swelling of the lips, tongue, or throat; difficulty breathing or swallowing; collapse; or a rapidly spreading purple or blistering rash. Seek prompt assessment for eye involvement, severe pain, pus or honey-coloured crusting, dehydration, unusual sleepiness, a stiff neck, or a fever with a child who looks very unwell. Arrange review if the rash persists, recurs, or changes after a new medicine or food.
