The fever may appear several days after vaccination rather than immediately. Monitor the child’s temperature and behaviour, offer fluids, and use fever medicine only as directed for the child’s age and situation.
What the fever may mean
The MMR vaccine trains the immune system against measles, mumps, and rubella. A mild fever can occur as the immune response develops and may come with tiredness, reduced appetite, or a light rash. This does not mean the child has measles or another vaccine-preventable illness.
A fever that starts very soon after the appointment may have another explanation, such as an infection already developing. The injection site can be sore, but marked redness, warmth, swelling, or discharge should be assessed because cellulitis is a different problem from an expected local reaction.
How to care for your child
Encourage frequent drinks and dress the child comfortably rather than wrapping them in extra layers. Check how they are responding, breathing, drinking, and urinating; these observations are more useful than a temperature reading alone.
Follow the vaccine record and the advice given at the appointment. Do not give aspirin to a child. If fever medicine is being considered, check the label or ask a pharmacist about the correct product and dose. Keep the child home from activities if they feel unwell.
When to seek medical help
Seek emergency help if the child has trouble breathing, swelling of the face or throat, becomes difficult to wake, has a seizure, develops a purple or rapidly spreading rash, or shows signs of severe dehydration such as very little urine and unusual drowsiness.
Contact a clinician promptly if the fever is worsening, lasts longer than expected, returns after settling, or occurs with persistent vomiting, a stiff neck, severe headache, or a child who is much less responsive. Mention the vaccination date and any other symptoms.

Ask how long the fever may reasonably last, which fever medicine is suitable, and whether the child’s symptoms fit a vaccine response. If a rash appears, ask what features would suggest chickenpox or another infection rather than a post-vaccination reaction.
