What Should You Know About Fever After Heat Stroke?

Heat stroke is a failure of the body's cooling system, not simply feeling warm after being outdoors. A raised temperature can continue while the brain and other organs are recovering. Confusion, unusual sleepiness, fainting, seizures, hot skin, a fast heartbeat, or poor coordination are especially concerning signs.

A temperature that remains high can also have another cause, such as an infection or a medicine-related reaction. The temperature reading and the person's mental state both matter, but a normal reading after cooling does not prove that the danger has passed. Recovery should be monitored by clinicians.

Call emergency services and say that heat stroke is suspected or has recently occurred. Move the person to shade or a cool room, remove excess clothing, and begin gentle cooling with cool wet cloths, fanning, or cool water while help is coming. Stay with them and keep checking their breathing and responsiveness.

Do not give drinks to someone who is confused, drowsy, vomiting, having a seizure, or unable to swallow safely. Do not rely on fever-reducing medicine to correct heat stroke; it does not remove the excess body heat and may be harmful in this setting. Clinicians may use monitored cooling and IV Fluid Therapy when appropriate.

Seek emergency care immediately for any fever or ongoing high temperature after suspected heat stroke, particularly with confusion, collapse, seizure, severe headache, breathing difficulty, chest discomfort, repeated vomiting, or very little urine. Do not drive an affected person yourself if they are confused or faint; emergency responders can start care during transport.

Even when the person seems better after cooling, arrange urgent medical evaluation. Heat-related injury can affect the kidneys, liver, muscles, and nervous system after the skin feels cooler. Tell the clinician when the heat exposure happened, what cooling was used, medicines or substances taken, and whether vomiting or loss of consciousness occurred.

Ask which temperature and symptoms should trigger another emergency visit, whether blood or urine tests are needed to check organ injury, and how to restart fluids and activity safely. Ask when it is safe to return to work, exercise, or outdoor heat.
