Fever after hand, foot and mouth disease

Hand, foot and mouth disease often causes fever before or around the time mouth ulcers and a rash appear. Temperature may settle as the child recovers. If it returns after improving, lasts longer than expected, or comes with worsening mouth pain, poor drinking, unusual sleepiness, breathing trouble, or a new rash, the cause may need checking. Fever itself does not identify one particular illness; other infections can occur at the same time.

Offer frequent small sips of water or an oral rehydration drink. Cool, soft foods may be easier when mouth ulcers hurt; avoid acidic or spicy choices. Keep the child lightly dressed and let them rest. Check whether they are urinating regularly and whether their mouth is moist. Use a fever medicine only as directed for the child’s age and weight, and never give aspirin to a child. Do not force food when fluids are the priority.

Seek urgent help if the child is difficult to wake, has trouble breathing, has a seizure, cannot keep fluids down, or shows signs of severe dehydration such as very little urine, a dry mouth, or marked weakness. Arrange prompt medical advice for a fever in a young infant, a fever that is worsening or returning, or a child who is drinking very little. A clinician can examine the mouth, skin, hydration, and other possible sources of infection.

Ask whether the fever pattern fits recovery, how much fluid the child should try to take, which medicine and dose are suitable, and which changes should prompt an urgent review.
