Fever accompanied by a rash

A fever and rash may occur with viral infections, including chickenpox or COVID-19, and can sometimes accompany a skin infection or a medicine reaction. Describe whether spots are flat, raised, blistered, bruised-looking, itchy, painful or rapidly spreading. Note where they began, whether they fade when pressed, and whether fever came before or after the rash. A photograph can help a clinician compare changes, but it does not replace an examination when someone looks ill.

Measure the temperature, record the timing and avoid scratching or applying a new cream until you have advice. Offer fluids and rest, and use suitable fever medicine only according to product directions. If an infectious illness is possible, limit close contact with pregnant people, newborns and anyone with reduced immunity until a clinician advises you. Tell the clinician about new medicines, foods, travel, insect bites and contact with someone who was unwell.

Get emergency help if the rash does not fade when pressed, looks purple or bruised, spreads quickly, or appears with a stiff neck, severe headache, confusion, breathing difficulty, facial swelling, fainting or a seizure. Seek prompt advice for a child who is unusually sleepy, dehydrated or difficult to settle, or for fever that is worsening. Phone ahead before attending if measles or chickenpox is possible so the clinic can advise about exposure.
