What should you know about febrile convulsions in infants?

Febrile convulsions are linked with fever in young children, but a very young infant may not show a clear temperature rise even when seriously unwell. Jerking, stiffening, sudden limpness, staring or unusual unresponsiveness can all be seizure-like signs. The appearance alone cannot identify the cause. A health professional will consider the infant’s age, temperature, recent symptoms, breathing, alertness and recovery. Infections such as influenza can cause fever, while other illnesses may require different treatment.

Put the infant on a firm, clear surface and move hard objects away. Do not restrain the body, shake the infant, or put anything in the mouth. Time the episode with a clock. When the movements stop, turn the infant onto their side if they are breathing normally and remain close while observing recovery. Do not give food, drink or oral medicine until the infant is fully alert. Contact urgent medical services for a first suspected seizure in a young infant, even if the episode has ended.

Call emergency services if the episode lasts longer than five minutes, repeats, affects breathing, or the infant becomes blue, very pale or difficult to wake. Seek urgent help for poor feeding, fewer wet nappies, repeated vomiting, a stiff neck, a non-fading rash, a weak cry, unusual sleepiness or persistent inconsolability. A temperature in a young infant should be discussed promptly with a clinician rather than managed only at home.

Ask what temperature and behaviour changes should prompt immediate help, whether the infant needs examination or tests, and how to record another episode. Confirm safe fever-medicine use for the infant’s age and weight, and request a clear plan for night-time symptoms.
