Familial Male Precocious Puberty

A boy may develop a larger penis, pubic or underarm hair, body odour, acne, a deeper voice, or faster growth earlier than peers. The pattern can occur in several male relatives, although family history does not prove that every early change has the same cause. The hormone signal may come from the testes rather than from the brain’s usual puberty pathway.

Early sex-hormone exposure can make bones mature faster. This may cause an early growth spurt while reducing the time available for further height later. A paediatric endocrinologist can assess growth over time, puberty signs, family history, and test results before discussing management.

Arrange a medical review if puberty signs appear unusually early or progress quickly. The clinician may examine growth and development, request hormone testing, assess bone age, and decide whether imaging is appropriate. Keep a record of when each change began and whether relatives had similar timing.

Management depends on the hormone pathway, the child’s growth pattern, emotional wellbeing, and the underlying genetic finding. Some children need observation, while others may be offered treatment to pause pubertal progression. Arrange prompt medical review for rapid progression, headaches, visual changes, seizures, severe abdominal or testicular pain, or a sudden change in behaviour. These features may require evaluation beyond familial hormone production. Seek emergency help for severe pain, collapse, or a seizure.
