What should you know about failure to thrive in neonates?

A neonate is a newborn in the first weeks of life. Clinicians consider birth history, expected early weight changes, subsequent gain, feeding effectiveness and hydration. Poor latch, tiring during feeds, illness, difficulty coordinating sucking and swallowing, or problems with digestion can affect progress. A single low reading does not explain the cause; repeated measurements and an examination help show the pattern. This concern is not a judgement about the person caring for the baby.

Contact the newborn’s maternity, paediatric or primary-care team promptly for advice. Record how often feeds occur, how long they last, whether swallowing is heard, vomiting, stools and wet nappies. Bring the discharge information and feeding plan. A clinician may observe a feed, check hydration, examine the baby and review weights. Follow the recommended feeding plan; do not change formula concentration or give water unless instructed.

Get urgent help if the newborn is hard to wake, breathless, blue or grey, has repeated forceful or green vomiting, has very few wet nappies, feels unusually floppy, or refuses several feeds. Fever or a clear fall in responsiveness also needs urgent medical attention in a newborn. If you are worried that the baby is deteriorating, seek emergency care rather than waiting for a routine appointment.
