Failure to thrive and growth charts

Measurements are plotted against reference curves for children of the same age and sex. Clinicians look at the child’s trend, the spacing between measurements, body proportions, and family pattern. A child may naturally track a lower or higher curve. Concern is greater when weight gain slows, measurements cross curves, or weight changes before length or height. Premature birth and medical history may change how early measurements are interpreted.

Keep a record of clinic measurements and ask how they compare with earlier points. Weighing repeatedly at home can create confusing results because scales, clothing, and timing differ. Share feeding details, appetite, activity, sleep, stools, and any illness with the clinician. The next step may be observation with a planned recheck, feeding support, or investigation of a specific symptom.

Contact a clinician promptly if a child is losing weight, drinking much less, vomiting repeatedly, having ongoing diarrhoea, or becoming less alert. Seek urgent help for breathing difficulty, severe dehydration, inability to keep fluids down, or a child who is hard to wake. The chart supports decisions but does not replace an examination.
