What should you know about facial scar revision surgery?

A surgeon first considers the scar's direction, depth, colour, firmness, movement, and relationship to facial features. Revision may involve removing a narrow segment and closing the skin carefully, releasing tethered tissue, or changing a contour. The purpose is usually improvement in visibility or function, not erasing the body's healing response. A revised scar still needs time to mature and may remain noticeable.

The timing matters. An active wound, infection, unstable scar, or untreated skin condition can change the plan. A raised or thickened scar may need a non-surgical approach, and a scar crossing an eyelid, lip, or joint-like facial fold may require particularly precise planning.

Bring details of the original injury or operation, previous scar treatments, allergies, medicines, smoking or nicotine use, and any history of raised scars. Ask how the incision will be placed in relaxed skin lines and what result is realistic for your scar. Follow instructions about cleansing, dressings, activity, and keeping the incision protected from sunlight.

During healing, mild swelling, bruising, tightness, tenderness, or itching can occur. Do not pick a scab or apply silicone products until the clinician says the wound is ready. Once the surface has healed, a silicone gel sheet may be discussed as part of scar care. Keep follow-up appointments because scar texture and colour change gradually.

Call the surgical team for bleeding that does not settle with their advised pressure, increasing pain, spreading redness, warmth, pus, wound separation, or fever. Get urgent help for facial swelling with breathing or swallowing difficulty, sudden vision change, or weakness affecting facial movement.

Ask which revision method fits the scar, where the new incision will lie, how long wound care may take, what could make the scar widen or thicken, and when silicone or massage is appropriate. Clarify anaesthesia, follow-up, activity limits, and options if the healed scar remains prominent.
