Face rash and fever: possible causes and next steps

Viral illnesses can cause fever with a widespread or facial rash. Chickenpox often begins with itchy spots that change over time. COVID-19, a common cold, or other respiratory infections may bring fever alongside cough, sore throat, or tiredness. A hot, swollen, painful area on one side of the face can fit cellulitis. A rash after a new medicine, food, or product may be an allergy.

Look at colour, texture, speed of spread, and whether pressing it briefly makes it fade. Also note breathing symptoms, mouth sores, eye symptoms, neck stiffness, and how alert the person seems.

Offer frequent fluids and rest, and use a cool compress for discomfort. Keep the skin clean without scrubbing, and avoid new creams, perfumes, and makeup. Do not pop blisters. Keep separate towels and wash hands if an infectious illness is possible. Ask a pharmacist or clinician about fever relief that is suitable for the person, especially for a child or someone taking other medicines.

Call emergency services for breathing trouble, swelling of the face or mouth, collapse, severe confusion, or a rash with a stiff neck or severe headache. Seek prompt assessment for purple or bruise-like spots, a rash that spreads quickly, severe pain, eye involvement, dehydration, or a person who is unusually difficult to wake. Arrange medical advice if fever persists, returns after improving, or the rash is not settling.
