What can cause it
Food poisoning and gastroenteritis can cause sudden watery stools with cramps, nausea, vomiting or fever. Food intolerance may produce a repeatable pattern after a particular food, while coeliac disease can cause longer-lasting digestive symptoms linked with gluten exposure. Irritable bowel syndrome may alternate between loose stools and constipation. Persistent diarrhoea can also occur with inflammatory bowel disease, giardiasis or an overactive thyroid. The pattern, travel history, medicines and associated symptoms matter more than stool looseness alone.
What to do at home
Take frequent sips of water and consider an oral rehydration solution if losses are substantial. Eat small, plain meals when able, and avoid alcohol and foods that clearly worsen symptoms. Wash hands carefully, do not prepare food for others while unwell, and note stool frequency, blood, fever, pain, recent antibiotics, travel and possible food exposures. Avoid anti-diarrhoeal medicine if there is blood, high fever or severe abdominal pain unless a clinician advises it.
When to seek care
Seek urgent care for confusion, fainting, inability to drink, very little urine, severe abdominal pain, bloody or black stool, or repeated vomiting. Arrange prompt review if diarrhoea persists, keeps returning, follows recent antibiotics or affects an infant, older adult or person with significant illness. A clinician may assess hydration and decide whether stool or blood tests are useful.
