Reading the pattern
Both hands and feet becoming cool in a cold room may be a normal vessel response. When the feeling continues in warm surroundings, look for accompanying clues. Anaemia or iron deficiency anaemia may present with tiredness, pale skin, dizziness or breathlessness. Hypothyroidism may present with cold sensitivity, low energy, dry skin or constipation. Peripheral arterial disease may present with foot or calf discomfort during walking, shiny skin, colour change, weak pulses or slow-healing wounds. Tingling or reduced sensation can make temperature harder to judge.
What to do
Warm the limbs gradually using socks, gloves and gentle movement; avoid placing numb skin directly against a very hot surface. Keep the skin moisturised and check toes and fingers for blisters or cuts. Make a routine appointment when symptoms persist, interfere with sleep or activity, or come with fatigue, colour changes, pain or numbness. Bring a list of medicines and note whether symptoms are equal on both sides.
When to seek care
Seek urgent care if a hand or foot suddenly turns pale, blue or black, becomes intensely painful or loses movement or sensation. Prompt assessment is also needed for a new wound that does not heal, rapidly increasing swelling, or severe pain at rest. Do not wait for warming measures to work when a limb changes suddenly.
