What should you know about extreme vulvar itching?

The vulvar skin can react to fragranced washes, wipes, pads, laundry products, tight clothing or hair-removal products. Contact dermatitis may cause itching, burning and redness where an irritant touched the skin. Skin yeast infection can cause intense itch with soreness, while psoriasis, atopic dermatitis or lichen planus can produce persistent inflammation or changes in texture and colour.

Scabies can involve severe itching, especially at night, and hives can cause raised itchy welts. Asteatotic eczema is another possible skin diagnosis, although the appearance and location matter. Itching alone does not identify one cause, and vaginal discharge, odour, sores or pain may change the evaluation.

Rinse with lukewarm water, pat dry, and stop fragranced products on the vulva. Wear loose breathable underwear, change out of damp clothing, and avoid scratching because broken skin can become more irritated. Do not put antiseptics, perfumes or unprescribed steroid or antifungal products inside the vagina.

Book an appointment if symptoms are intense, recurrent or unexplained. A clinician may inspect the skin and take a swab when infection is possible, then choose treatment based on the finding rather than the itch alone.

Seek prompt care for blisters, open sores, bleeding, marked swelling, fever, pelvic pain, painful urination, or new symptoms after sexual contact. Urgent assessment is appropriate for rapidly worsening swelling, severe pain, or difficulty passing urine.

Ask whether the skin looks irritated, infected or inflamed; whether testing is needed; which products to stop; and how to relieve the itch without damaging delicate skin. Tell the clinician about discharge, periods, pregnancy, sexual exposure, new products and previous treatments.
