What should you know about extreme ringworm?

Ringworm is a fungal infection, not a worm. It often produces an expanding, scaly rash with a more active edge and clearer skin toward the centre, although appearance varies by skin tone and treatment. It can itch, crack, or become inflamed, and scratching may spread fungal material to nearby skin. Steroid creams used alone can mask the border and allow the infection to persist.

A clinician may inspect the rash and, when the diagnosis is uncertain, collect surface scale for testing. Other rashes can resemble ringworm, so worsening after self-treatment is a reason to reassess rather than keep adding products.

Keep affected skin clean and dry, wash hands after touching it, and avoid sharing towels, clothing, razors, or sports equipment. Wash fabrics that contact the rash and clean items that stay damp. Topical antifungal treatment may be suitable for limited skin infection, while oral antifungal treatment may be considered when infection is widespread, recurrent, or in a location where cream is inadequate. Use the prescribed product for the full advised course and follow the label directions.

Seek prompt medical care for rapidly spreading redness, marked swelling, pus, fever, severe pain, involvement of the scalp or nails, or infection in someone with reduced immunity. Arrange review if the rash continues to enlarge, returns repeatedly, or remains scaly after treatment. A painful, crusted rash may need a different diagnosis or additional care.

Ask whether the rash is consistent with ringworm or needs testing, which body sites are involved, and whether topical or oral treatment is more suitable. Ask how to protect household members, whether pets could be a source, and when it is reasonable to return to close-contact activities.
