Short answer

Extreme postnatal depression is a serious, treatable illness after birth that needs prompt support, especially when safety or reality feels affected.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Mood, sleep, thinking, bonding and daily care

What extreme postnatal depression can involve

Symptoms may include persistent sadness or emptiness, severe worry, irritability, guilt, loss of interest, trouble sleeping even when the baby sleeps, or difficulty feeling connected. Some parents struggle to eat, concentrate or manage ordinary care tasks. Thoughts of self-harm, harming the baby, or being unable to stay safe require immediate attention. Hearing or seeing things others do not, feeling watched, or becoming unusually confused can signal a psychiatric emergency rather than depression alone.

Low mood after birth may occur alongside reduced sexual desire; this does not mean either symptom has one single cause. A clinician can assess mood, sleep, physical recovery, medicines and support at home.

What to do next

Tell a trusted person today what you are experiencing and ask them to stay with you if you feel unsafe or overwhelmed. Contact your maternity team, family doctor or mental health service promptly. Perinatal mental health support can include talking treatment, practical support and medicines chosen with breastfeeding and your health in mind. Accept help with meals, rest, feeds and appointments, and avoid alcohol or non-prescribed drugs while symptoms are severe.

When to seek urgent care

Seek emergency help now if you may harm yourself or your baby, cannot care safely, feel detached from reality, hear commands, or are severely confused or agitated. Do not stay alone; ask someone to take responsibility for the baby while you get help. If danger is immediate, call local emergency services or go to the nearest emergency department. Urgent assessment is appropriate even if you are unsure whether the symptoms are severe enough.

Questions for your doctor

Which symptoms suggest depression, anxiety or a postpartum psychiatric emergency? What support is available today? Which treatments fit my feeding plans and medical history? How can we make a safety plan, and who should my family contact if my symptoms change?

Questions people ask

Can postnatal depression begin after the first weeks?

Yes. Symptoms can begin later after birth or become more intense over time, so new or worsening changes deserve assessment.

What if I am afraid to tell someone?

Share the facts with a trusted person or clinician anyway. Severe symptoms are medical concerns, not a failure as a parent, and early support helps protect you and your baby.
