What should you know about extreme knee swelling?

Short answer
Extreme knee swelling needs attention because fluid, bleeding, inflammation, or injury can restrict movement and signal a problem requiring assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common clue
- Sudden swelling after injury differs from gradual swelling with stiffness.
- Until assessed
- Rest, elevate, and use a wrapped cold pack; avoid forceful movement.
What extreme swelling may mean
Swelling may develop inside the joint or in the tissues around it. A sudden increase after a twist, fall, or sporting movement can accompany an ACL Tear or Meniscus Tear. Gradual swelling with stiffness can occur with Knee Osteoarthritis. A painful, warm flare may occur with Gout, while localized swelling over the kneecap can fit Knee Bursitis. A soft fullness behind the knee may be a Baker's Cyst. The timing, temperature of the skin, range of motion, bruising, and ability to bear weight help a clinician narrow the cause.
Do not force the knee through a blocked movement. Severe swelling can alter how you walk and place extra strain on the hip, ankle, and other knee.
What you can do now
Stop the activity that triggered the swelling and rest the leg in a comfortable position. Use a cold pack wrapped in cloth for short periods, elevate the leg, and avoid tight wrapping that causes numbness or colour change. Gentle movement within a comfortable range can help prevent stiffness, but do not massage a freshly injured or very hot knee. A clinician may examine the joint and, when appropriate, consider Joint Aspiration to remove fluid and clarify what is causing the swelling.
When to seek care
Arrange prompt medical assessment if swelling is extreme, appeared suddenly, follows an injury, or makes weight-bearing difficult. Seek urgent help for a hot red knee with fever or feeling unwell, a visibly deformed joint, new numbness or weakness, severe pain after a fall, or a foot that becomes pale, blue, or cold. These features need timely examination rather than home treatment alone.
Questions for your doctor
Ask: Is the fluid inside the joint or in the surrounding tissue? Could the pattern fit an ACL Tear, Gout, or another cause? Do I need imaging or fluid testing? Which movements are safe while it settles, and what symptoms should make me return sooner?

Questions people ask
Can extreme knee swelling go away on its own?
Some mild swelling settles with rest, but extreme, sudden, hot, or persistent swelling should be assessed to identify the cause.
Should I drain a swollen knee myself?
No. Fluid removal is a clinical procedure, and the cause of the swelling should be checked first.
