What should you know about extreme ankle swelling?

Short answer
Extreme ankle swelling needs prompt assessment when it is sudden, severe, painful, or accompanied by colour, temperature, or breathing changes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Useful clues
- Onset, injury, heat, redness, pain, and weight-bearing ability.
- Until assessed
- Elevate and avoid painful loading.
- Emergency signs
- Breathing trouble or a cold, pale, blue, or numb foot.
What it means
Swelling is fluid collecting in or around the ankle. A recent twist can cause an ankle sprain; a fall or direct impact can cause an ankle fracture. A hot, red, very tender joint can occur with gout or infection. Longer-term stiffness and swelling may occur with ankle osteoarthritis. One-sided swelling without an obvious injury, or swelling with calf symptoms, needs a clinician to consider causes beyond the joint. The appearance alone cannot identify the cause.
What to do now
If the swelling followed a minor injury, protect the ankle, rest from painful loading, elevate it, and use a wrapped cold pack for short periods. Avoid tight wrapping if toes become numb, pale, or cold. Do not walk through severe pain or try to force a stiff joint. Arrange prompt assessment if you cannot bear weight, the ankle looks deformed, or swelling is rapidly increasing. A clinician may examine the joint and decide whether imaging or other tests are needed.
When to seek care
Seek emergency help for ankle swelling with chest pain, breathlessness, fainting, a blue or unusually pale foot, a cold foot, or new loss of sensation. Same-day assessment is needed for severe pain, fever, spreading redness, an open injury, deformity, or inability to bear weight. Sudden swelling without injury also deserves prompt medical review.
Questions for your doctor
Ask what pattern the examination suggests, whether an X-ray or other test is needed, how much weight you may place on the ankle, and which warning signs should change your follow-up plan.

Questions people ask
Can a sprain cause marked swelling?
Yes, a sprain can cause swelling and bruising, but severe pain, deformity, or inability to bear weight needs assessment for a fracture or another problem.
When is ankle swelling urgent?
It is urgent when it is sudden, rapidly worsening, very painful, hot and red, linked with fever, or accompanied by breathing or circulation changes.
