Exercises for an MCL sprain

How the structures involved in Ligament Sprain differ from normal

Short answer

Exercises for an MCL sprain should begin with gentle motion and progress to strength and balance work as pain and stability improve.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Early focus
Gentle motion and comfortable muscle activation
Progression
Strength, balance, and controlled side-to-side demands

What the injury means

The medial collateral ligament supports the inner side of the knee. A sprain can make turning, side-stepping, kneeling, or walking uncomfortable. Early rehabilitation aims to keep the knee moving without provoking pain, while later work helps the thigh and hip muscles control the knee.

How to exercise safely

Start with comfortable knee bends, heel slides, and gentle thigh tightening while seated or lying down. Add straight-leg raises when you can lift the leg without the knee sagging. As walking becomes easier, supervised sit-to-stand practice, supported step-ups, calf raises, and single-leg balance can build control. Keep movements slow, use support near you, and stop if pain becomes sharp, swelling increases, or the knee feels less stable. A physiotherapist can adjust the load and may recommend a brace or taping for selected activities; see [Physiotherapy](/treatments/physiotherapy-and-rehabilitation/physiotherapy/) or [Kinesiology Taping](/treatments/physiotherapy-and-rehabilitation/kinesiology-taping/). Splinting may be considered when protection is needed; learn about [Splinting](/treatments/trauma-and-fracture-care/splinting/).

When to seek care

Arrange an assessment if you cannot bear weight, the knee gives way repeatedly, swelling is substantial, or the injury followed a forceful twist. Seek prompt care for a locked knee, visible deformity, numbness, a cold foot, or rapidly worsening pain. A clinician can check for associated meniscus, cartilage, or ligament injuries before you increase exercise.

Questions for your doctor

Ask which movements match the severity of your sprain, whether a brace is appropriate, how to judge a safe increase in activity, and which signs mean you should pause rehabilitation.

Questions people ask

Can I exercise with an MCL sprain?

Often, gentle exercises are introduced within comfort, but the timing and load depend on knee stability and injury severity.

Should exercise hurt?

Mild effort can be expected, but sharp pain, increasing swelling, or giving way means the activity should stop and be reviewed.
