Episiotomy care during postpartum recovery

The postpartum period combines wound healing with vaginal bleeding, hormonal changes, tiredness, and the demands of caring for a newborn. An episiotomy can make sitting, walking, urinating, or bowel movements uncomfortable. Healing should gradually become easier, although the pace differs between people. The scar may feel firm or itchy as it settles.

Keep the area clean with water and dry it gently. Change pads often and wash your hands before and after toilet care. Sit in a position that reduces pressure, rest when possible, and return to activity gradually. Use pain relief only as directed by your maternity clinician. Fluids, fibre, and prescribed constipation treatment can make bowel movements easier. Avoid sex or anything in the vagina until bleeding has settled, the wound is comfortable, and you feel ready; discuss contraception and lubricant if needed.

Arrange prompt assessment for pain that is increasing rather than easing, new swelling, redness, pus, foul odour, fever, or a visible gap in the stitches. Seek urgent help for heavy bleeding, fainting, severe weakness, or new loss of bladder or bowel control. A postpartum review is also appropriate when scar pain or painful sex persists.
