Epidermal Melasma: What to Know

Short answer
Epidermal melasma is a form of facial hyperpigmentation in which extra melanin sits mainly in the outer layer of the skin.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Appearance
- Flat brown or grey-brown facial patches
- Helpful habit
- Consistent light protection with gentle skin care
How epidermal melasma appears
It commonly creates flat brown or grey-brown patches on areas such as the cheeks, forehead, nose, or upper lip, often with a fairly even surface. Sunlight can deepen the colour, and heat, hormonal changes, or irritation may also influence it. The epidermal location can make pigment more visible under examination, but pigment depth and diagnosis are best assessed by a clinician. Other causes of dark patches can look similar, including post-inflammatory colour change and sun-related skin change, so a new or unusual mark should not be labelled melasma from a photograph alone.
Managing the colour
Use broad-spectrum sunscreen and physical protection such as a hat, shade, or careful timing outdoors; reapply sunscreen as directed on its label. Choose gentle, fragrance-free products and stop products that sting or cause persistent redness. A dermatologist may discuss prescription pigment treatments or procedures such as a chemical peel, microneedling, skin brightening treatment, or laser toning. These options can irritate skin and may worsen pigmentation in some people, so the plan should match your skin tone, triggers, and diagnosis.
When to seek care
Arrange an assessment if pigmentation appears suddenly, changes quickly, itches, bleeds, becomes raised, or forms an irregular single spot. Seek review when darkening continues despite careful light protection, or when a treatment causes burning, blistering, swelling, or prolonged redness. Pregnancy, breastfeeding, medicines, and sensitive skin can change which options are suitable.
Questions for your doctor
Ask whether the pattern fits epidermal melasma, what triggers may be relevant, and how to protect the skin from visible light and ultraviolet exposure. Discuss realistic time frames, maintenance, side effects, and how to pause treatment if irritation develops.

Questions people ask
Is epidermal melasma raised?
Melasma is generally flat rather than a bump. A raised, bleeding, or rapidly changing mark needs separate assessment.
Can a peel or laser remove melasma permanently?
Procedures may improve colour for some people, but pigment can return when triggers persist, and treatment needs careful selection.
