What happens to the lining

The endometrium is the lining inside the womb. During reproductive years it responds to hormones and sheds during a period. After menopause, hormone stimulation falls and the lining generally becomes thin. An ultrasound may describe its appearance, but an image alone does not explain every cause of bleeding. Polyps, hormone medicines, vaginal or cervical changes and other conditions can also be relevant.

What an assessment may involve

Tell a clinician about any spotting or bleeding, even if it happened once. Share when your last natural period was and list hormone medicines, tamoxifen or blood thinners if relevant. Assessment may include an examination, ultrasound and sometimes a sample of the lining. Keep a note of colour, amount, duration and associated pain; this makes the consultation more useful.

When to seek care

Arrange prompt medical review for bleeding after menopause, bleeding after sex, recurring spotting or pelvic pain. Seek urgent help if bleeding is heavy, you feel faint or weak, or pain is severe. Do not wait for hot flushes or other menopausal symptoms to return before reporting bleeding.
