Itchy ears inside with a sore throat

Itchy ears with a sore throat can occur when a cold, allergy, reflux, or throat infection irritates connected areas of the nose, throat, and ear passages. Blocked ears, sneezing, a runny nose, cough, fever, or swollen glands can help distinguish the pattern, but symptoms alone cannot identify one cause.

Allergic rhinitis can inflame the nose and the tube that helps equalise pressure behind the eardrum, causing itching or fullness alongside throat irritation. A common cold, influenza, COVID-19, tonsillitis, or strep throat may cause a sore throat with referred ear discomfort. Acid reflux can irritate the throat, especially after meals or when lying down, without causing a typical cold pattern. Glandular fever is another possibility when marked tiredness and swollen neck glands accompany the throat symptoms.

Drink fluids, rest, and avoid smoke or other throat irritants. Do not put cotton buds, drops, or objects into an itchy ear unless a clinician recommends them; scratching can damage the ear canal. Saline nasal rinsing may help some people with nasal symptoms when used as directed. Consider a medical review if symptoms persist, recur, affect hearing, or follow swimming or ear trauma.

Seek urgent help for difficulty breathing or swallowing, drooling, severe dehydration, sudden hearing loss, swelling behind the ear, severe ear pain, or a high fever with rapid deterioration. Arrange prompt assessment for pus or blood from the ear, facial weakness, or a sore throat that is strongly one-sided.

Ask whether the ear canal and eardrum need examination, whether allergy or reflux treatment may help, whether a throat swab is appropriate, and which changes should prompt urgent review.
