Ear piercing tear repair: what to know

A piercing tear can be a small split, an elongated hole, or a complete division of the lobe. It may happen when jewellery catches, is pulled, or places ongoing weight on the tissue. The clinician examines bleeding, contamination, infection, scar tissue, and the remaining lobe shape. A recent injury may need wound care first; a settled tear is usually assessed as a planned repair.

For a new tear, apply gentle pressure with clean gauze if it is bleeding and avoid putting jewellery back through the opening. Do not use harsh antiseptics inside the wound. After repair, keep the site clean and dry as directed, protect it from snagging, and attend the planned review. Avoid sleeping on the lobe or placing headphones over it if that causes pressure.

Get prompt assessment for uncontrolled bleeding, a deep or contaminated wound, increasing warmth and redness, pus, fever, or worsening pain. Urgent care is appropriate if the lobe turns pale or blue, becomes rapidly swollen, or you feel seriously unwell. A wound that opens after closure also needs review.

Ask whether the tear should be repaired immediately or after swelling settles, how the piercing tract will be handled, and what scar care is suitable. Clarify when you can wear earrings, whether re-piercing is possible, and how to reduce the chance of another tear.
