Ear infection without fever: what to know

Fever is one possible response to infection, not a requirement. An infection in the ear canal or middle ear may produce local pain, pressure, blocked hearing or fluid without changing body temperature. Ear discomfort can also follow a cold, congestion, allergies, water exposure or wax buildup. A normal temperature cannot identify the cause or show whether the eardrum and canal look healthy.

Rest and drink fluids, and consider suitable pain relief according to the label or professional advice. Keep water out of the ear and avoid inserting buds, fingers or home remedies. Note whether pain is one-sided, whether hearing is muffled, and whether fluid appears. Arrange an examination if pain persists, interferes with sleep, or is not clearly improving.

Seek prompt care for discharge, worsening pain, swelling around or behind the ear, new dizziness, facial weakness, severe headache, or a sudden hearing change. Urgent help is needed for confusion, breathing difficulty, inability to swallow fluids, or a child who appears seriously unwell. A person with diabetes, immune suppression or recent ear surgery should seek advice sooner.

Ask whether the infection is in the canal or behind the eardrum, whether fluid is present, how to protect hearing, and what review time is appropriate. Confirm which pain medicines are suitable for the person’s age and health.
