What thick, dry toenails can mean

A thick nail may become rough, crumbly, yellow, white, brown, or separated from the nail bed. Fungal infection is one possibility, especially when several nails or nearby skin are affected, but it is not the only explanation. Repeated pressure from footwear, an old injury, and nail psoriasis can create similar changes. A single altered nail after trauma may grow out differently from a gradually worsening group of nails. The visible nail cannot reliably identify the cause, and treatment for fungus will not help every thick nail.

Safer nail care

Keep feet clean and dry, change damp socks, and choose shoes with enough room for the toes. Trim nails straight across after bathing when they are softer, and do not dig into the corners or share clippers. File only loose rough edges and stop if there is pain or bleeding. Avoid covering a suspected infection with cosmetic products until it has been assessed. A clinician may examine nail debris or recommend testing before selecting a topical, oral, or device-based treatment.

When to seek medical care

Arrange an assessment if thickening persists, spreads to other nails, causes pressure in shoes, or comes with discoloration, debris, or separation. Seek prompt care for swelling, warmth, pus, an open wound, severe pain, or a dark streak that is widening or reaching the surrounding skin. People with diabetes, poor circulation, reduced sensation, or immune problems should seek advice early for foot or nail changes.
