Understanding a shoulder knot
A knot often describes a tender, tense area in muscles such as the upper trapezius, levator scapulae, or muscles around the shoulder blade. A clinician checks whether the pain changes with muscle pressure, shoulder motion, neck motion, or nerve testing. Needling can be used in a selected muscle to produce a brief local response and reduce sensitivity.
This does not show that muscle tension is the only cause. Myofascial Pain Syndrome can include painful muscle bands, while shoulder pain may also relate to a tendon, joint, neck nerve, or recent overload. A clear examination matters before choosing a needle location.
Getting ready and looking after the area
Wear a top that permits access to the upper back and shoulder. Tell the physiotherapist about blood-thinning medicine, easy bruising, skin infection, pregnancy, fainting with needles, or an implanted device. During treatment, mention dizziness, sharp pain, or symptoms travelling into the arm. Afterwards, keep the shoulder moving gently and complete the prescribed mobility or strengthening work.

When to seek care
Arrange a review if shoulder pain is steadily worsening, arm function is declining, or a treatment site becomes hot, red, swollen, or increasingly painful. Seek urgent help for chest pressure, breathlessness, fainting, sudden arm weakness, severe neck pain after injury, or new loss of coordination.
