Dorsal hand swelling: what to know

The dorsal hand is the back of the hand, where swelling can make the knuckles, tendons, or fingers look puffy. A fall onto the hand or wrist can cause swelling with tenderness and reduced movement; a Distal Radius Fracture may also affect the nearby wrist and hand. Swelling with warmth, prolonged morning stiffness, or several painful joints can occur with Rheumatoid Arthritis. Sudden redness, heat, fever, a wound, or a bite raises concern for infection or another urgent problem.

Remove rings early if swelling is increasing. Rest from heavy gripping, keep the hand comfortably elevated, and use a cool pack wrapped in cloth for short periods after a recent injury. Do not force stiff fingers or massage an area that is hot, very painful, or possibly fractured. Gentle finger movement can help preserve mobility when it is comfortable. A clinician can decide whether the hand needs imaging, joint assessment, or treatment for inflammation.

Get prompt medical advice after a fall, direct blow, or twisting injury if swelling is substantial, the hand looks unusual, or movement is limited. Seek urgent help for blue, pale, cold, or numb fingers; rapidly increasing swelling; severe pain; an open wound; fever; or spreading redness. Persistent or repeated swelling also warrants assessment, particularly when several joints are stiff or painful.
