Does physiotherapy treat rotator cuff tendinitis?

Short answer

Physiotherapy is commonly used for rotator cuff tendinitis to restore shoulder movement, strengthen the cuff and adjust loads that keep irritating the tendon.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the condition involves

Rotator cuff tendinitis, also called rotator cuff tendinopathy, can cause pain when reaching, lifting or lying on the affected side. The tendon may become less tolerant of repeated or sudden loading. Assessment helps distinguish tendon-related pain from problems involving the neck, a tear or other shoulder structures.

A physiotherapy programme often starts with comfortable movement and load modification, then adds isometric or resistance exercises as tolerance improves. It may also address shoulder-blade movement, posture, work technique and sleep positions. Improvement is usually gradual, and exercises should be adjusted if pain remains more intense after activity or movement becomes increasingly restricted.

How to get started

Arrange an assessment if shoulder pain is affecting dressing, work, sport or sleep. Reduce repeated overhead activity for now, but avoid keeping the arm still all day unless instructed. Follow the prescribed exercises at the recommended level and note which loads are comfortable. A therapist can help you return to lifting by increasing demand in small, controlled steps.

When to seek care

Seek prompt medical advice for sudden weakness after an injury, inability to lift the arm, marked swelling, a deformed shoulder or a fever with a hot, red joint. Urgent help is needed for chest pain, severe breathlessness or a cold, blue or numb hand. Arrange reassessment if pain steadily worsens or motion continues to decline.

Questions for your doctor

Ask whether your symptoms fit tendinitis or another shoulder problem, which activities should be reduced, and how to progress resistance exercises. Discuss pain relief options, sleep positioning and the point at which imaging or specialist review should be considered.

Questions people ask

Should I keep exercising with rotator cuff tendinitis?

Often, modified movement and graded strengthening are useful, but the exercise level should be adjusted when symptoms flare or remain worse afterwards.

How is tendinitis different from a tear?

Tendinitis or tendinopathy involves an irritated or less load-tolerant tendon, while a tear is a structural split; examination may be needed to tell them apart.
