Does lactose intolerance cause abdominal pain?

Short answer
Most people with Lactose Intolerance notice abdominal pain after consuming lactose, often with gas, bloating or loose stools.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical trigger
- Milk and other lactose-containing foods
- Common timing
- Often within a few hours of eating dairy
- Useful record
- Food, portion, timing and symptoms
What the pain can mean
Lactose is the natural sugar in milk and many dairy foods. When the small intestine does not make enough lactase, lactose moves into the large bowel without being fully digested. Bacteria break it down and produce gas, while extra water can enter the bowel. This can create cramping, pressure or a dull ache, commonly within a few hours of dairy.
The pattern matters more than one episode. Pain that repeatedly follows milk, ice cream or soft cheese and eases when lactose is reduced supports discussing lactose intolerance with a clinician. Abdominal pain has many other possible causes, so this symptom alone cannot identify the cause.
What you can do
Keep a short food-and-symptom diary, noting the dairy food, portion, timing and symptoms. A clinician may suggest a planned reduction of lactose followed by a careful reintroduction, or arrange testing when appropriate. Lactose-free milk and yoghurt may be easier to tolerate, and some people use lactase products with dairy after checking the label and asking a pharmacist.
Avoid cutting out all dairy for a long period without advice. Milk and dairy provide nutrients such as calcium, so ask about suitable alternatives if you reduce them.
When to seek care
Arrange medical advice if pain keeps returning, disrupts sleep or daily activities, or comes with ongoing diarrhoea, vomiting, weight loss or blood in the stool. Seek urgent help for severe or rapidly worsening pain, a swollen and hard abdomen, fainting, confusion, or inability to keep fluids down.
Questions for your doctor
Could my timing of pain after dairy fit lactose intolerance? Do I need a breath test or another assessment? How can I reduce lactose while meeting calcium needs? Could a different digestive condition explain these symptoms?
