Does cauda equina syndrome cause bladder or bowel changes with back pain?

Bladder or Bowel Changes With Back Pain is a very common presentation of Cauda Equina Syndrome. This symptom pattern needs urgent assessment because compression of the lower spinal nerves can affect bladder, bowel and leg function.

Cauda equina syndrome involves pressure on the bundle of nerves at the lower end of the spinal canal. Most people with Cauda Equina Syndrome notice bladder or bowel changes with back pain. Changes can include difficulty starting urine, reduced awareness of bladder filling, new leakage, loss of bowel control or difficulty passing stool. Numbness around the inner thighs, buttocks or genital area, and weakness in one or both legs, can occur as well. Other conditions can cause individual bladder, bowel or back symptoms, so an examination is needed to identify the cause.

If these changes are new or worsening, go to an emergency department now or contact emergency services. Do not wait for a routine appointment, arrange a massage, or drive yourself if leg weakness or numbness makes driving unsafe. Tell the clinician when the bladder or bowel change began, whether sensation has changed, and whether weakness, numbness or severe back pain is present.

Seek emergency care immediately for new difficulty passing urine, new loss of bladder or bowel control, numbness in the saddle area, or worsening weakness in the legs alongside back pain. The combination can require rapid assessment and imaging. A sudden inability to walk, severe pain after injury, or fever with severe back pain also warrants emergency evaluation.
