Does benign prostatic hyperplasia cause difficulty passing urine?

As the prostate enlarges, it can press around the urethra, the tube that carries urine out of the bladder. This may make the stream weak, cause hesitancy, or require straining to begin. Some people feel that the bladder has not emptied fully. Symptoms can develop gradually and may be more noticeable when the bladder is very full.

Difficulty passing urine has other possible causes, including infection, medicines, stones, or narrowing of the urethra. The symptom alone cannot identify the cause, so an assessment may include questions about timing, flow, pain, and medicines, followed by an examination or urine testing.

Arrange a medical review if the change is persistent, worsening, or interfering with sleep and daily activities. Keep a brief note of when the problem occurs, whether the stream stops and starts, and whether there is burning, blood, fever, or dribbling. Review medicines with a clinician before changing them; some cold remedies and other medicines can worsen urinary difficulty. A clinician can discuss monitoring, medicines that relax the prostate area, or other treatment when appropriate.

Seek urgent medical help if you cannot pass urine at all, especially with painful lower-abdominal swelling. Prompt assessment is also needed for fever or chills with urinary symptoms, severe pain, vomiting, or visible blood in the urine. These features can signal retention, infection, or another problem needing timely treatment.

Ask what may be narrowing the urine flow, whether your medicines could contribute, and which tests can distinguish prostate enlargement from infection or another blockage. You can also ask how quickly symptoms should improve and which warning signs should prompt an urgent review.
