Dizziness in older adults: what to know

Short answer
Dizziness in older age may reflect blood pressure changes, medicines, dehydration, inner-ear problems or illness, and falls or new neurological symptoms require prompt care.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- First response
- Sit down and steady yourself
- Review to request
- Medicines, standing blood pressure and fall risk
Possible reasons
Some people feel faint after standing because blood pressure does not adjust quickly. Others describe spinning when turning in bed, which can occur with benign paroxysmal positional vertigo. Poor fluid intake, heat, infection, low blood sugar or anaemia can also affect steadiness. Heart rhythm conditions such as atrial fibrillation may cause palpitations or faintness. Several medicines taken together can add to the problem, so a medication review is useful.
Stay steady and reduce falls
Sit down immediately when symptoms begin and use good lighting, handrails and supportive footwear. Stand in stages: move to the edge of the bed, pause, then rise with a stable surface nearby. Keep water accessible and avoid walking alone during an episode. Ask a clinician or pharmacist to review medicines, including recently added tablets and over-the-counter products. Do not make medication changes independently.
When to seek urgent care
Call emergency services for sudden weakness, facial drooping, speech difficulty, severe headache, chest pain, fainting, confusion or a new inability to walk. Seek prompt review after a fall, head injury, repeated near-faints, persistent vomiting or dizziness with a racing or irregular heartbeat.
What to ask
Could standing blood pressure or my medicines be contributing? Should my balance, hearing, blood count, hydration or heart rhythm be checked? What changes at home would reduce fall risk?

Questions people ask
Why can standing cause dizziness in older adults?
Blood pressure may take longer to adjust when changing position, and medicines, dehydration or illness can make that response less effective.
Is dizziness after a fall urgent?
A fall with a head injury, fainting, confusion, severe pain or new weakness needs urgent assessment, even if the person later feels better.
