Difference Between Spotting and Breakthrough Bleeding

Spotting describes a small amount of vaginal bleeding, while breakthrough bleeding is bleeding between expected periods, often linked with hormonal contraception. The terms can overlap: breakthrough bleeding may look like spotting, but breakthrough bleeding refers mainly to when the bleeding happens and its relationship to a contraceptive method. A heavier flow between periods is still unexpected bleeding and deserves attention if it is new, persistent, or troublesome.

Spotting is light staining or bleeding that may appear on toilet tissue or underwear rather than filling a pad. It can occur around ovulation, before or after a period, after sex, during early pregnancy, or with changes affecting the cervix or uterus. Breakthrough bleeding is bleeding that breaks through the expected pattern of bleeding while using a hormonal contraceptive such as a pill, implant, injection, patch, or hormonal intrauterine device. It can happen while the body adjusts, after missed or late doses, or when a method does not suit someone. Infection, cervical changes, uterine growths such as endometrial polyps, and pregnancy-related causes can also produce bleeding between periods.

Record when the bleeding starts, how long it lasts, its approximate amount, its colour, and any pain, discharge, fever, or bleeding after sex. Note the contraceptive method, recent changes, and any missed or delayed doses. Follow the product instructions and do not double doses or stop a prescribed method without checking with a clinician or pharmacist. If pregnancy is possible, take a pregnancy test according to the test instructions. Arrange a routine appointment if the pattern continues, returns repeatedly, happens after sex, or concerns you; an assessment may include a pregnancy test, infection testing, and an examination based on your symptoms.

Seek urgent medical care for bleeding with severe or one-sided lower abdominal pain, fainting, marked dizziness, shoulder-tip pain, or a positive pregnancy test. Seek prompt advice if bleeding is heavy, you feel weak or unwell, you have fever or worsening pelvic pain, or bleeding follows sex and does not settle. These symptoms can need assessment for pregnancy-related problems, infection, or another cause. If you have reached menopause, any vaginal bleeding should be assessed, even when it seems like spotting.
