Diastasis recti coning postpartum: what it means and what to do

How the structures involved in Diastasis Recti differ from normal

Short answer

Postpartum coning is a visible ridge or dome along the abdominal midline when pressure rises, often linked with diastasis recti and reduced coordination of the deep core.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common trigger
Sit-ups, lifting, coughing, or getting up
Useful focus
Breathing, pressure control, and gradual loading

What coning can show

The abdominal wall stretches during pregnancy. When you sit up, lift your baby, cough, or exercise, pressure inside the abdomen can push the midline connective tissue forward. A ridge does not by itself measure the size or severity of a separation, and it is not a diagnosis of a hernia. Notice whether the shape appears with a particular movement and whether it comes with pain, heaviness, or loss of control.

Many people can improve pressure management and abdominal function after birth. Recovery is individual, especially after a caesarean birth, pelvic-floor symptoms, or a long period away from exercise.

Practical next steps

Start with comfortable breathing and gentle movements that let the abdomen stay supported rather than forcefully bulging. Roll to your side before getting out of bed, exhale during effort, and temporarily reduce exercises that repeatedly produce a prominent cone. Build activity gradually, using symptoms and control as your guide.

A pelvic-health clinician can observe your movement, check the abdominal wall and pelvic floor, and adapt a programme to feeding, lifting, work, and sleep demands. Postnatal Physiotherapy can help with this assessment and progression.

When to seek care

Arrange an assessment if coning persists with ordinary tasks, abdominal or back discomfort limits you, or you notice pelvic pressure, urine leakage, or difficulty lifting. Seek prompt medical care for sudden severe pain, vomiting, a tender lump that will not soften when you lie down, or a change in skin colour over a bulge.

Questions for your clinician

Could this ridge be diastasis recti, a hernia, or another abdominal-wall issue? Which daily movements should I modify? How will you judge progress besides the appearance of the midline? When can I return to running, lifting, or higher-load exercise?

Questions people ask

Is coning after birth dangerous?

Coning is often a sign that a movement is creating more pressure than you can currently control, but the appearance alone does not establish danger. Symptoms and examination matter.

Should I stop all exercise?

Usually, no. Modify movements that cause repeated doming and choose comfortable activity while rebuilding control.
