Diagnosing an MCL sprain

How the structures involved in Ligament Sprain differ from normal

Short answer

Diagnosing an MCL sprain involves the injury history, a focused knee examination, and imaging when the severity or another knee injury needs clarification.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Location
The MCL is on the inner side of the knee
Assessment
Tenderness, motion, stability, walking, and associated injuries are checked

What the clinician checks

The medial collateral ligament runs along the inner side of the knee. A blow to the outside of the knee or a twisting force can strain it. The clinician asks how the injury happened, then checks inner-knee tenderness, swelling, range of motion, walking, and whether the knee feels unstable when gently stressed. They also assess the other knee structures because more than one tissue can be injured in the same event. A sprain may be stretched or partly torn; examination helps estimate its functional severity.

Steps after assessment

Protect the knee from twisting and impact until it has been assessed. Use the support recommended for your symptoms and keep movement within a comfortable range. An X-ray may check for a bone injury, while MRI can show ligament and cartilage structures when the examination suggests a more complex injury. Physiotherapy commonly focuses on restoring motion, then strength, balance, and control. Splinting or Kinesiology Taping may be discussed as supportive options for selected patients.

When to seek care

Arrange prompt review after a forceful knee injury with inability to bear weight, marked swelling, locking, obvious deformity, or a feeling that the knee gives way. Seek urgent help if the foot becomes cold, pale, numb, or unusually weak. Persistent instability, worsening pain, or difficulty straightening the knee deserves reassessment even if the original swelling has settled.

Questions for your doctor

Ask whether the MCL is the only injured structure, what level of protection is needed, and whether imaging would change management. Discuss safe exercises, expected milestones, and whether Physiotherapy, Splinting, or Kinesiology Taping fits your examination findings.

Questions people ask

Can an MCL sprain be diagnosed from swelling alone?

No. Swelling has several possible causes. The injury mechanism, inner-knee tenderness, stability testing, and sometimes imaging are considered together.

Does an MCL sprain always need a brace?

Not always. The need for support depends on stability, pain, function, and other injuries found during assessment.
