What should you know about dermatophyte onychomycosis?

Short answer
Dermatophyte onychomycosis is a fungal nail infection that can cause thickening, crumbling, or altered colour; diagnosis and treatment should account for the nail and the surrounding skin.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How dermatophyte nail infection appears
Dermatophytes are fungi that can invade the nail plate and the tissue beneath it. An affected nail may become yellow, white, or brown, thicker than usual, brittle, crumbly, or separated from the nail bed. The infection can occur with athlete’s foot because fungi may spread between the skin and nails. Not every discoloured nail is fungal: trauma, psoriasis, inflammation, and other conditions can look similar. A clinician may examine nail clippings or scrapings before choosing prolonged treatment.
Nail growth is slow, so improvement is judged by healthy nail growing from the base rather than by an instant change in the damaged portion. Several nails, the nail root, or surrounding skin may affect the treatment plan.
Treatment and prevention
Keep feet clean and dry, change damp socks, wear breathable footwear, and avoid sharing nail tools. Treat coexisting athlete’s foot as advised, because untreated skin infection can reseed the nail. Topical antifungal treatment may suit selected limited infections, while oral antifungal treatment may be considered when disease is more extensive or involves areas topical medicine cannot reach. Laser treatment for nail fungus has a different role and should be discussed with a clinician. Nail trimming or nail surgery may be considered for a painful, severely thickened, or mechanically troublesome nail.
When to seek care
Arrange an assessment for a new persistent nail change, pain, swelling, drainage, or separation from the nail bed. Seek prompt care if redness spreads around the nail, pus develops, or walking becomes difficult. People with diabetes, poor circulation, reduced sensation, or a weakened immune system should seek advice early when a foot or nail problem appears, because a small injury can become harder to manage.
Questions for your doctor
Ask whether the nail change has been confirmed as fungus, whether a sample is needed, which nails and skin areas should be treated, and how long to continue therapy. Discuss possible medicine interactions and monitoring if oral treatment is considered. Ask how to recognise healthy regrowth and when a persistent nail might need nail surgery.

Questions people ask
Can a discoloured nail be diagnosed from appearance alone?
Appearance can suggest fungus, but trauma, psoriasis, and other nail conditions can look similar, so confirmation may be recommended before treatment.
Why does treatment take patience?
The damaged nail does not instantly become normal; improvement is seen as clear, healthy nail gradually grows from the base.
