What a questionnaire assesses
Questions commonly explore mood, interest, energy, sleep, appetite, concentration, movement, guilt and thoughts about death or self-harm. You are usually asked to describe how often or how strongly symptoms affected you during a defined period. Answer as honestly as you can; there is no useful score to achieve.
A result is one part of an assessment. A clinician also considers your history, functioning, physical health, medicines, substance use, stressors and safety. Fatigue, insomnia, daytime sleepiness and difficulty concentrating can have several causes. A questionnaire can support a discussion, track change over time or prompt a fuller assessment.
Using the result well
Complete the form in a quiet moment and ask what time period it covers. Tell the clinician if symptoms fluctuate, began after a medicine change, or are connected with a menstrual cycle, illness or major stress. Bring questions about sleep, anxiety, irritability and daily functioning, because the score may not capture every concern.
Do not use an online score to reassure yourself that serious symptoms are harmless or to choose medicine. If treatment is recommended, ask what it involves, when it should be reviewed and what changes should prompt contact. A repeat questionnaire can show movement, but your lived experience remains central.
When to seek urgent help
Arrange medical care when symptoms persist, worsen or interfere with work, relationships or self-care, even if a questionnaire result seems low. Seek immediate help if you have thoughts of suicide, intend to harm yourself, cannot stay safe, or feel severely confused or disconnected from reality. Tell someone you trust and contact local emergency services when danger is immediate.
