Depression and insomnia: what to know

Short answer
Depression and insomnia can reinforce each other, so treatment should address mood, sleep habits, medicines and any contributing health problem.

How mood and sleep interact
Insomnia can mean difficulty falling asleep, waking often, waking too early or sleeping without feeling restored. Depression may bring low motivation, worry, slowed thinking and changes in the body clock; these can make bedtime harder. In turn, broken sleep may leave you with Fatigue, Daytime Sleepiness, Irritability or Difficulty Concentrating. Some people sleep more rather than less. A clinician will consider mood symptoms, sleep timing, caffeine, alcohol, pain, breathing problems, other medicines and periods of unusually high energy. Keeping a sleep and mood diary can reveal patterns.
Steps that may help
Keep a steady wake time, reserve the bed for sleep, and use a quiet wind-down routine. Get daylight and gentle activity during the day, while limiting late caffeine and long naps. If you remain awake for a long time, move briefly to a dim, calm activity and return when sleepy. Book an appointment if symptoms persist, reduce daily functioning or follow a medicine change. Treatment may include structured psychological therapy for insomnia, depression care, or both.
When to seek urgent care
Seek urgent help if depression includes thoughts of suicide, self-harm, inability to stay safe, severe agitation, hallucinations or several nights with almost no sleep alongside unusually elevated energy. Seek medical advice promptly for daytime sleepiness that makes driving unsafe or for breathing pauses during sleep.
Questions for your doctor
