What should you know about Demodex mites on the human scalp?

Short answer
Demodex mites can live in scalp follicles without causing symptoms, but an increased mite burden or skin sensitivity may contribute to itching, redness, scaling or follicle irritation.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What scalp Demodex may mean
Demodex are microscopic organisms associated with hair follicles and sebaceous glands. Their presence alone does not prove that they are causing a scalp problem. Symptoms can overlap with dandruff, seborrhoeic dermatitis, psoriasis, folliculitis or irritation from hair products. A clinician may examine the scalp and, when needed, inspect a skin sample rather than relying on appearance alone.
Scalp involvement may feel like crawling, tingling or itch, with flaky skin, redness or small inflamed bumps. Scratching can break the skin and make irritation worse. Demodex on the scalp does not mean that mites are living along the length of each hair.
What you can do now
Use a gentle cleanser and rinse away styling products thoroughly. Avoid picking, vigorous scratching, harsh scrubs and essential oils placed directly on the scalp. Wash pillowcases, hats and hair tools regularly, and do not share combs or brushes while the scalp is irritated.
If symptoms persist, a dermatologist can distinguish mite-related inflammation from other scalp conditions and select treatment accordingly. A prescribed or recommended [Medicated Shampoo Treatment](/treatments/medical-dermatology/medicated-shampoo-therapy/) may be considered when a medicated wash fits the diagnosis; follow its directions and keep it away from the eyes.
When to seek care
Arrange a dermatology assessment for persistent itch, spreading redness, painful bumps, crusting, discharge or noticeable hair thinning. Seek prompt medical help if swelling is severe, the scalp becomes very painful, or you feel unwell with a rapidly worsening rash. These signs can reflect infection or another condition that needs direct examination.
Questions for your doctor
Ask whether the findings fit Demodex-related irritation or another scalp disorder, whether testing is useful, which ingredients to avoid, and how long to use any medicated wash. Mention recent hair treatments, new products, medicines and whether anyone else in the household has similar symptoms.

Questions people ask
Can Demodex mites cause scalp itching?
They may contribute to itching or follicle irritation in some people, but dandruff and several other scalp conditions can feel similar.
Should I shave my hair?
Shaving is not a dependable way to address follicle-associated mites. Gentle cleansing and an assessment of the underlying inflammation are more useful.
