What should you know about delivering twins vaginally?

Your maternity team reviews the babies’ positions, growth, placentas, your health and the pregnancy stage. A head-down first twin can make vaginal birth more feasible, but the second twin may change position after the first is born. Monitoring helps the team respond to changes in the babies’ heart rates or your labour. Birth usually takes place where urgent obstetric, anaesthetic and newborn support is available.

Discuss the proposed birth plan before labour, including pain relief, monitoring, the possibility of assisted birth and circumstances in which a caesarean section may be recommended. Attend planned scans and appointments so growth and position can be reviewed. Pack practical support for recovery and arrange help at home, because caring for two newborns can make rest and feeding more demanding.

Contact your maternity unit urgently for heavy bleeding, severe or persistent abdominal pain, regular painful contractions, fluid leakage, reduced movement of either baby, severe headache or vision changes. Call emergency services if you feel faint, have severe breathing difficulty, or think a baby is about to be born.

Ask how each baby is positioned, where delivery is recommended, what monitoring will be used, and which findings would change the plan. You can also ask about pain relief, recovery expectations and whether a Growth Scan is needed.
