Short answer

Delayed puberty age refers to puberty starting later than expected for a child’s sex and development pattern, and timing should be judged alongside growth and family history.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How timing is judged

Puberty is not a single event. Clinicians look for a sequence of changes, including breast or testicular development, pubic hair, growth acceleration, voice change, and menstruation where relevant. A child may simply have a later family pattern, especially when growth and general health are otherwise reassuring.

Concern is greater when there are no physical changes by the expected developmental stage, growth has slowed, or puberty begins but does not progress. Possible contributors include low nutrition, long-term illness, hormone conditions, or differences affecting the ovaries, testes, brain, or thyroid. A delayed period alone can have several explanations.

Planning an evaluation

Keep a record of height, weight, growth changes, and the first signs of puberty. Share family history, medical conditions, medicines, eating patterns, and exercise with the clinician. An evaluation may include a physical examination, growth-chart review, hormone tests, and a bone-age X-ray. Further testing is chosen according to the findings rather than used automatically for every child.

When to seek care

Arrange a routine review if puberty has not started by the expected stage, if development has paused, or if there is no expected menstrual development. Seek earlier advice for headaches, vision problems, pronounced tiredness, unexplained weight change, persistent abdominal pain, or symptoms of chronic disease. Prompt assessment is also reasonable when delayed development is causing distress or bullying.

Questions for your doctor

Ask which developmental milestone is absent, whether the growth pattern suggests a family tendency, what tests may clarify the cause, and how often follow-up is needed. Ask when treatment might be considered and what emotional or school support is available.

Questions people ask

Is there one exact age when puberty must start?

There is a normal range, so clinicians consider the sequence and progress of changes rather than one birthday alone.

Should a delayed period always be investigated?

A missed or absent period can have different causes; medical review is useful when expected development has not occurred or other symptoms are present.
